REMINDER FOR YEAR 9 PARENT/CARERS

The immunisation team will be at Charlton school on the 6^th and 7^th of July for the following vaccines - Meningococcal ACWY and DIPTHERIA, TETANUS AND POLIOMYELITIS

Please return your consent form as soon as possible via reception or email – school@charlton.uk.com. This must be returned even if you do not consent for your child to be vaccinated.
